here's the deal with the rubber cap: usually the shift knobs have a certain 
universal diameter hole to fit all shifters.

The rubber stopper usually comes in a couple of sizes and sits on the end of 
the shifter and is supposed to make up the gap so that the knob can't wiggle 
around.

The screws alone won't hold it securly if it's not a snug fit - mine came 
loose every other day. The best thing to do is to find some rubber tubing (I 
used the clear pvc tubing like they use for airhorns), measure the hole in 
the knob, and cut a piece to lenght. Then cut strips out of the tubing so it 
will fit around the shifter completely. Use a piece of tape etc to hold them 
in place, slide the knob down and tighten the srews.

Basic idea, place rubber around the shifter to keep it from pivoting at the 
point where you screw it down.

I got my new momo knob today but I'm unsure on one thing with the
installation.  Basically the rubber cap that goes on. Do I put that on and
shove the knob on so it goes all the way inside the knob and then put the
scews in to clamp on the the stick itself, or does the knob just sit on the
rubber cap and the screws clamp into the rubber cap?  If the last way is
correct then I have to cut my shift stick.  Any help would be appreciated.
The included instructions don't show how far into the knob the rubber cap
goes.  Thanks.
