hesitation at part throttle at 3k

this one kinda stumped me. i recently got my motor running good again. and
then a few days later, the car would jerk back and forth at part throttle at
3k. now if i stomp on it, the hesistation would go away. at first i thought
it was a vaccum leak, i slapped a gauge on and im getting a good 18-19 at
idle. so.. i'm not sure what the hell is going on now.. i'm figured it might
be a fuel delivery problem. i changed all the injectors yesterday, i
adjusted the mixture a bit. but still no go. while the hesitation is
happening, the car seems to be running lean, i'm watching my air/fuel gauge,
and it's leaning out while the hesitation is happening.

any suggestions?
