What can be done to help a tall driver fit?

The knee deal is an ongoing problem for me. There are steering wheel hub
extenders available that can be used with aftermarket wheels; both are on my
wish list.

The trend in flat-bottomed steering wheels will also help, if you're OK with
the styling.
