problem with throttle cable / gas pedal connection......STILL!

ok, i got the rubber/foam grommet that goes in the hole of the gas pedal and
holds the cable in place.

problem is, it doesn't hold it in place. the rivet head just slips right
through the grommet. are there more parts that i need to add? the other
thing is that when it is connected, the cable is at an angle instead of
going straight back through the firewall beceause of the way it's
attatched....so if i flor it, then back off, the throttle sticks and the
engine is throttled to about 2200 rpms....

since the grommet wouldn't completely hold it in, i tried using a washer
that is blistered up a bit, so you can push it over the rivet head....then a
couple of washers to hold that on the gas pedal.

this thing is really pissing me off, any ideas as to what the hell i might
have done wrong....or am missing?
