clutch failure

clutch is not disengaging all the way. possible causes:
- carpet in way of clutch pedal
- excessive free play in clutch cable (pedal hits floor before clutch is
disengaged)
- rubber bushings at tranny clutch arm are torn/giving
- throwout bearing issue (or adjacent)
- one of the pressure plate bands has broken (plate then rides on a bevel
around the clutch, never fully disengaging).
