Fuel pump?

ok.  i've had this problem for quite some time now.  i'm guessing it is the 
in-tank fuel pump, but want to know for sure before shelling out the money.  
when i hit a quarter of a tank (seems almost exactly) the car will hesitate 
for a bit then run fine.  then with further driving under 1/4 tank, it will 
evenly hesitate again horribly...to the point were you have the car floored 
and nothing happens...then it gets gas, accelerates, then no more gas...it 
does this until you shut the key off and turn it back on.  then it's fine 
for a while until it happens again...the cycle repeats.  also it will 
hesitate at times under hard acceleration (even with like 3/4 tank) when 
shifting into 4th and 5th gear, or down-shifting into 4th(not as often).  
and finally the biggest PITA is during traffic jams when the car gets warm 
(with nearly full tank) which i don't know how this would affect the in-tank 
fuel pump, since only the engine gets hot.  i'm guessing the all started 
when i nearly ran it out of gas a while back, but not sure since the systoms 
didn't start immediately following that.  although they have gotten 
progressively worse.  so what should i be looking for?  TIA!
