You mentioned many ignition items, but, humor me, did you change the
Coil?

As I may have mentioned before, the coil was my culprit, and on mine
there was a ground wire going from the Coil mounting bracket to the
cold start injector.

Coil, Coil, Coil.........

-or- since youve done everything else, this is doubtful but the
ignition module which is mounted under/around the windshield wiper
motor.

BTW: here is the logic as I see it.  A coil when cold is not as potent
(just like your battery).  When cold the 5th injector kicks on and you
crank away--BUT because the Coil is weak the spark is weak, and you
are in effect drowning/flooding with too much gas.  When you unplug
the 5th, the weakend spark is not as overwhelemed and is able to fire.
