Heel & Toe

Heya:

I'm not a racer (I usually obey the speed limit--who else can claim
that?) but I'm interested in heel and toeing. I thought that the standard
way is to have the ball of the foot on the brake while the heel or edge of
the foot was used to push the accel. I find this hard to do in my '81.
It seems that the gas pedal is positioned too high relative to the brake,
and even though I have small feet (size 8.5) they seem to get blocked by
the center console/heater housing when I try to pivot my foot far enough to
reach the gas. On the other hand, although it's awkward, it seems like
it's possible to brake with the heel while pivoting the ball of the foot
over the gas.
What do you autoxers do? Are there pedal covers to extend the gas
downward? Do you wear dainty little ballet slippers? Have squid-like
mobility in the ankle region? Any input is appreciated, as they say.
