Flange cap seals

I replaced both flange seals on the FN tranny. the outer big ones. I =
also replaced both the caps/seals that reside inside the flanges. But =
the one on the drivers side had a hole in it,it was machined on the =
joint side . My mechanic had replaced that cap 20K miles ago. Is it =
possible he put the wrong one? I thought there was only "one size fits =
all",
What would cause that to happen ? improper installation?
